I wasn't entirely sure how much of the terribleness of trans healthcare is gatekeeping and outright transphobia, and how much is 'just' administrative badness. I think some of it is that gender-affirming treatment doesn't seem like an emergency to cis planners, but if all admin is just paperwork and red tape that's going to mess up even the kind of treatment that is considered urgent.
